Element Care is a nonprofit healthcare organisation founded in 1994, dedicated to providing comprehensive, coordinated care for adults aged 55 and older. The organisation's mission is to help older adults live safely and independently in their own homes, a model that emphasises person-centred support and community engagement.
The centrepiece of its services is the PACE Program, a Medicare- and Medicaid-funded model that bundles primary care, nursing, physical and occupational therapy, social day programmes, care coordination, vision services, meals and transportation into a single, all-inclusive offering. This integrated approach is designed to reduce the complexity of navigating fragmented healthcare systems for elderly patients and their families.
Element Care serves a defined geographic area covering all of Essex County, the Merrimack Valley and parts of Greater Boston, including the North Shore. Its operating philosophy is rooted in a neighbourhood-oriented ethos, reflecting a commitment to taking care of local communities.
