Community Care Plan (CCP) is a health plan organisation founded in 2000 and headquartered in Florida. Its core business is serving Medicaid recipients and participants in the Florida Healthy Kids programme, with a stated commitment to promoting healthier communities.
The organisation's operations span several distinct health plan offerings. These include Medicaid (MMA) plans available across 19 Florida counties and Florida Healthy Kids plans for children aged 5-18 in eight counties. Beyond these, CCP manages self-insured employee health plans. The organisation also operates SydCura, a division handling uninsured programmes and value-based care initiatives, and 22 Health, which provides Affordable Care Act plans to Broward County residents via the health insurance marketplace.
